SMHS ATHLETIC CLEARANCE
SMHS SPORTS PHYSICALS: Students who wish to participate in sports activities during the 2022/23 school year will need to have a Sports Physical.
PHYSICAL CLEARANCE PACKETS: Students who wish to participate in a sport for the 2022/23 school year you will need to have a Physical Clearance Packet completed and turned into the Front Office before you can practice. Screenings may only be performed by a licensed MD, DO, PAC, and NP. Also, screening must be performed after June 1st, 2022 for the 2022/23 school year participation. Sports Physical Packets may be printed from the website, see below. Packets will also be available in the front office.
