The Applications, Description of Benefits and Claim Forms will be available on the link below. to access the application on line, please follow the steps listed below:

SMHS ATHLETIC CLEARANCE
PHYSICAL CLEARANCE PACKETS: Students who wish to participate in a sport for the 2023/24 school year you will need to have a Physical Clearance Packet completed and turned into the Front Office before you can practice. Screenings may only be performed by a licensed MD, DO, PAC, and NP. Also, screening must be performed after June 1st, 2023 for the 2023/24 school year participation. Sports Physical Packets may be printed from the website, see below. Packets will also be available in the front office.

Screenings may only be performed by a licensed MD, DO, PAC, and NP. Also, screening must be performed after June 1st, 2025 for the 2025/26 school year participation.
